Create Smart eForm¶
- Select the
Formstab - Press the 'Create Form' button as shown in the following image:

- Fill in the appropriate fields
Form Fields
| Field | Description |

| Name | The name for the Smart eForm. |
| Is Active | Controls if the Smart eForm is available for use. |
| Expiry Date | Controls the date that the Smart eForm will expire and no longer be available for selection. |
| Description | General description of the Smart eForm. |
- Click the
Savebutton.

Form Version¶
A Smart eForm needs to have a Form Version.
- Click the
+button on theForm Versionlist to create a new version as per the following image:

Form Version Fields
| Field | Description |

| OVERVIEW | |
| Form | The Parent Form |
| Version | The current version number |
| Status | The current Status of the Form Version. This is automatically controlled when you activate a Form Version. This will start with the value 'Draft'. When activated it will change to 'Active' and if another version is activated the previous versions become 'Inactive'. |
| Auto Activation Date | The date the Form Version will automatically become 'Active'. Previous Form Versions will become 'Inactive' and will no longer be able to be used for a new Form Instance. Note: If this field is not provided the form will not automatically activate. This needs to be set using the Set Auto Activate button. |
| Activation Date Time | The date/time the Form Version became 'Active'. |
| Deactivation Date Time | The date/time the Form Version become 'Inactive'. |
| Reviewer Team | The Team responsible for Reviewing the Form Instance if applicable. |
| Approval Team | The Team responsible for Approving the Form Instance if applicable. |
| On Complete Team | The Team the Form Instance should be assigned to when the form is completed. |
| Child Only | Controls if the Form must have a related parent record as defined in the Available Parent Types property. If set to false there is no need for the form to have a parent. If set to true the form needs to be related to a parent record as defined in the Available Parent Type property. |
| Available Parent Types | The list of Parent Entity Types that the Form can be related to. Supported values are:

| Enable Sign Off | Controls if the Form requires sign off to be completed and enables you to select the Users required to sign off the form. |

| LAYOUT OPTIONS | |
| Header Main Label | Text to display in the Main Header Label. Refer to the example image below. |
| Header Sub Label | Text to display in the Sub Header Label. Refer to the example image below. |
| Progress Bar Enabled | Controls if a Progress Bar is displayed in the Smart eForm to help the user understand their current progress through the form questions. |
| Progress Bar Label | Label that can be displayed in the Progress Bar. |
| Submit Message | The final message that is displayed to the user when the form is saved. Note: This is a mandatory field. It could be as simple as 'Thank you for completing the form'. |
Note
The 'Auto Activation Date' is set using the 'Set Auto Activate' button that becomes available once the 'Form Version' is saved.
Main & Sub Header Labels¶
The following shows an example of defining a value in the Header Main Label and Header Sub Label. This is always displayed at the top of the Smart eForm.

Progress Bar¶
The following is an example of the Progress Bar and a Progress Bar Label. The progress bar shows current progress through the Smart eForm as a percentage of completion.
This is only displayed if the Progress Bar Enabled property is set to true. This is always displayed under the header section of the Smart eForm as shown in the previous image.

Activating a Smart eForm Version¶
In order to use a Smart eForm you need to Activate the appropriate Smart eForm Version. Depending on your experience with creating Smart eForms and effort put into designing the process flow before jumping in and creating the Smart eForm will largely affect the number of different versions that you end up creating as you test each change.
To activate an eForm version, click to a version to navigate to its Details View and press Set Active button.

Revising a Smart eForm Version¶
Form Versions can be revised anytime. Once revised, the status will be defaulted to Draft. Active and Inactive Form Versions are not editable, when you wish to amend them, you need to Revise a version back to Draft.
Click Revise button in the Details View of a form version to revise it.
